Life Insurance $50,000 policy through Canada Life Insurance Company paid by MSB&A. Ninety day eligibility period.
Additional Life Insurance

Paid by employee through payroll deduction. Provided by Canada Life Assurance Company. Coverage limits in $10,000 increments from $10,000 to $500,000. Limit of five times the employee's annual earnings. AD&D coverage is included in same amount of life coverage.
Spouse benefit - One-half (50%) of employee amount, up to $250,000. AD&D is included in same amount of life coverage.
Children benefit - Each child, 14 days and older is eligible for insurance in the amount of $10,000 per child.

Long Term Disability Policy paid by MSB&A provided through Northwestern Mutual. This policy pays 66.67% of your basic monthly earnings beginning the ninety-first day after the date you become disabled. Maximum benefit - $5,000.00 per month, minimum benefit - $100 or 10% of your maximum benefit, whichever is greater. Ninety day eligibility period.
Health Insurance MSB&A contributes a flat rate towards the premium for employee and dependent coverages which equals approximately 65% of the premium for the employee and 50% of the dependent premium. Two plans are available through Aetna U.S. HealthCare. Plan 1 is the Open Choice PPO with a Calendar year deductible of $500.00 per person, $1,500.00 per family, $15.00 co-pay for physician visits within the network, and a three tier Rx drug card ($10.00 Generic/$20.00 Brand/$35.00 non-formulary) at participating pharmacies. Out of the network reimbursement is at 60%. Plan 2 is the Selected Choice HMO, $15.00 co-pay for Primary Care Physician (PCP) visits within the network, $20.00 co-pay for Specialty and Hospitalization & Surgery, and three tier Rx drug card ($10.00 Generic/$20.00 Brand/$30.00 non-formulary) at participating pharmacies. No out of network reimbursements is available. Ninety day eligibility period.
Voluntary Dental Plan Pay by the employee through payroll deduction. Provided through Aetna U.S. HealthCare's Advantage Dental Program. Once a primary care dentist is selected, you will be provided dental services at a rate of $5.00 per office visit for most preventative and diagnostic services such as routine checkups, cleanings, X-rays, etc. No deductible or annual dollar maximums. Ninety day eligibility period.
   


Flexible Time Off (FTO)

This is an annual account of time from which you may draw paid time off to cover personal needs such as vacations, sick/personal days, holidays, etc. as follows:

Nine Core Holidays which are observed under FTO are: New Year's Day, Mardi Gras Day, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Day after Thanksgiving, Christmas Day.


MSB&A 401(k) Plan

MSB&A matches 10% of the first 5%. To be eligible for this plan, you must be continuously employed by MSB&A for at least one year, and at least 21 years of age.


Direct Deposit

All employees are eligible to elect that their paycheck is directly deposited into their checking and/or savings accounts.
